ABOUT THE ROLE
Under the direct supervision of the Manager – Client Flow and Staffing, the Coordinator – Hospital to Home (H2H) is responsible for coordinating safe, effective, and person-centered transitions for clients from hospital discharge to home. The Coordinator ensures timely follow-up, efficient care coordination, and connection to appropriate services to support recovery, independence, and reduced risk of Alternate Level of Care (ALC) and hospital readmission. The Coordinator works to ensure seamless transitions, minimizes gaps in care and optimizes client outcomes. Furthermore, the Coordinator H2H is responsible for coordinated and efficient systems that better serve the needs of clients and their families across the continuum of care. This role aligns with ALC Leading Practices, Home First philosophy, and system flow priorities, supporting improved outcomes for older adults across the continuum of care.

WHAT YOU BRING
Education/Experience:
- Regulated health professional in good standing (e.g., Social Worker, Registered Nurse, Occupational Therapist, Physiotherapist, or other relevant designation)
- Current registration with the applicable professional regulatory college
- Minimum five (5) years of experience in a related healthcare field such as geriatrics
- Demonstrated experience in change management, including leading and implementing projects
Skills/Abilities:
- Proven ability to work effectively with older adults with complex medical, cognitive, and social needs
- Knowledge of geriatric syndromes, frailty assessment, restorative care principles, ALC Leading Practices, and the Home First philosophy
- Strong clinical assessment, counseling, and client/family education skills
- Excellent verbal and written communication skills
- Demonstrated critical thinking, problem-solving, and conflict resolution abilities
- Ability to work both independently and collaboratively within an interdisciplinary team
- Familiarity with community resources and navigation across the continuum of care
- Proven ability to engage stakeholders and build effective partnerships and coalitions
- Expertise in geriatric assessment and identification of frailty
- Demonstrated systems thinking with the ability to support patient flow optimization across care settings
- Commitment to client and family-centred care
- Strong interprofessional collaboration skills
- Demonstrated application of Home First philosophy and ALC Leading Practices
- Adaptability and ability to intentionally build and sustain partnerships and networks
- Demonstrated regular attendance at work
Conditions of Employment:
- This position shall be conditional upon a satisfactory Criminal Record & Judicial Matters Check (CRJMC) where indicated, to ensure the absence of relevant criminal convictions.
- Will be required to travel to various work locations and to provide own transportation
