ABOUT THE ROLE
Reporting to the Manager, Regional Referral System the Referral Coordinator will support Central Intake for the North West Wound and Vascular pathway. The Referral Coordinator is responsible for routing referrals after initial assessment to all required services based on the care plan determined by the Advanced Practice Clinician in Wound Care. The North West Wound and Vascular pathway uses a distributed model of Advanced Practice assessors located across North West Ontario and supports a principle of care as close to home as possible. As such, this pathway connects clients with approximately 30 services and over 300 services locations across North West Ontario.
This position requires a system-wide orientation as clients may be assessed and care plans created by agencies other than St. Joseph’s Care Group. The Referral Coordinator will also contribute to system improvement and support planning activities by identifying service gaps and shortages across the network.

WHAT YOU BRING
Education/Experience:
- Bachelor's degree or Diploma in health care administration, registered health care professional or a related field
- At least 2 years of experience in a healthcare environment
Skills/Abilities:
- Excellent analytical, critical thinking and deductive reasoning skills to interpret information, and identify specific areas that require further investigation
- Effectively document all relevant events in a timely and efficient manner as cases develop with an advanced attention to detail
- Extremely well-honed organizational skills to prioritize day-to-day activities to allow efficient and effective progress of file management activities
- Ability to multitask and deal effectively with ambiguity in a high time sensitive and fast paced environment
- Be self-directed, goal-oriented and able to manage time and priorities with minimal supervision
- Experience with system navigation and knowledge of community resources
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to establish and maintain effective working relationships with superiors, co-workers, and customers
- Ability to identify, understand, and meet the requirements of internal and external customers
- Independent judgment when dealing with confidential and sensitive issues, including the ability to maintain confidentiality of records, actions, and information relating to clients and/or hospital business
- Proficient in data entry with the ability to maintain a high degree of speed and accuracy
- Ability to switch between and work across multiple digital platforms is required
- Demonstrated regular attendance at work
- High degree of proficiency and comfort with technology including Electronic Medical Records and electronic databases
- Knowledge of the health care system in North Western Ontario including acute and post acute care, primary care, home care, long term care and community support services
- Demonstrated regular attendance at work
Asset:
- French Language proficiency is considered an asset
- Knowledge of referral management software is considered an asset
Conditions of Employment:
- This position shall be conditional upon a satisfactory Criminal Record & Judicial Matters Check (CRJMC) where indicated, to ensure the absence of relevant criminal convictions.
